Frank Schneider Memorial Muskie Tournament 2-Day Family Entry

REGISTRATIONS MUST BE SUBMITTED BY 10 PM SEPTEMBER 7.

Two adults (Family Head and spouse/significant other) from the same household and up to 2 juniors OR
One adult and up to 3 juniors

If you have only one adult and one junior, it will save you entry fees to register each separately with an Adult/Junior entry for each.

Only fill in address for other family members if it is different from Family Head. All juniors must include their birth date. Email and phone are required for each entry.

Each adult MUST sign (check box) for Release of Liability. Parent/guardians must sign a paper copy of Release of Liability for juniors. It will be emailed to you.

ZONE 7 and ZONE 8 Parking
Hubbard County requires a parking plan at lakes in their county. It is against state laws to take more that 50% of parking spaces for tournaments. If you selected ZONE 7 or ZONE 8 you must indicate where you will park your vehicle and/or trailer during fishing hours.

TOURNAMENT ID/PACKET PICK-UP – Select pick-up location on Friday, September 11. Must be there between 7-9 pm.(Required)

AQUATIC INVASIVE SPECIES/BOAT STATUS(Required)

Indicate the status of the boat you will be fishing from during the tournament or any pre-fishing. If you are the owner of the watercraft, you acknowledge that it is your personal responsibility to abide by all state laws regarding the transport of aquatic invasive species.

The watercraft in which I will participate in this tournament does not belong to me.

By checking this box, I acknowledge that I am aware of the inherent risk associated with fishing and boating, including but not limited to the risks of collisions with other boats or stationary objects in and around the lake, including rocks, reefs or piers or other vehicles and trailers at landing areas. I am familiar with other risks up to and including death by drowning associated with rough weather, changing weather patterns, heavy winds and lightning storms. I expressly assume all risks associated with this tournament and I hereby release Muskies, Inc., the Twin Cities Chapter #1 of Muskies, Inc., sponsors and tournament officials from all claims of negligence resulting from injury and/or property damages incurred in connection with the tournament.
